Informed Consent and Acknowledgement

Sunshine Sports Terms of Registration;

PARENT/GUARDIAN MUST AGREE TO DISCLAIMER / Terms of registration BEFORE PARTICIPATION

Your Permission:

I give permission for my child / registered participant to attend the Sunshine Sports Football programs/clinics and will not hold Sunshine Sports, its staff, sponsors/partners, or volunteers responsible for damage and or loss of property and or accident and or injury and or death or illness.

I also give permission for an ambulance & medical attention should the need arise. The parent/guardian gives permission to Sunshine Sports to take photos & videos of the participant for the purpose of promotional material & social media use including but not limited to Facebook, Instagram, and Twitter.

I the parent also agree not to send my child if:

• They have any cold or flu symptoms
• Have been instructed by NSW Health to self-isolate.

The Parent/Guardian & player acknowledge all fees are strictly non-refundable. Fees will ONLY be refunded if the application is not accepted. *

If you do not wish to agree with any of the terms above please confirm this in writing prior to completion of your registration.

Discount Code Policy:

Discount codes provided through partnerships (e.g., Marsden Park Galaxy FC) are exclusively for use by registered members of the partnering organisation. Misuse of these discount codes, including but not limited to sharing codes with non-members or unauthorised use, may result in the cancellation of the registration and forfeiture of any fees paid. Sunshine Sports reserves the right to verify eligibility for discounts with the partnering organisation.

Confirmation: BY ACKNOWLEDGING AND AGREEING BELOW, I AM DELIVERING AN ELECTRONIC SIGNATURE THAT WILL HAVE THE SAME EFFECT AS AN ORIGINAL MANUAL PAPER SIGNATURE. THE ELECTRONIC SIGNATURE WILL BE EQUALLY AS BINDING AS AN ORIGINAL MANUAL PAPER SIGNATURE.
